To open the Lead Source Entry form, click on Setup in the drop-down menu, then click on Lists, then click on Lead Source.

The Lead Source form allows you to create Lead Sources. Lead Sources are used in the lead information you enter in the "Lead / Follow-Up Info" tab of the Edit Job form.
